Membership Agreement

Version 1.0 · Effective October 30, 2026

This agreement is between The 63 Day Transformation LLC, doing business as The Profit Lifestyle ("we"), and you, the member. Checking the box at checkout is your signature.

  • Access to the Live Forever Health clinic platform and its medical directors, so you can place provider-prescribed orders for your clients

Investment

$597 per month. Your card is charged today and on the same date each month after.

Cancel any time

Email [email protected] with 30 days' notice. No refunds for partial months.

Terms

  1. Billing. You authorize us to charge your card monthly until you cancel. If a payment fails and isn't fixed within 10 days, we may pause your access. We'll give you 30 days' notice before any price change.
  2. Cancellation. Your access continues through the 30-day notice period, and any billing date inside it is still charged. No refunds for partial months or unused calls.
  3. Clinic platform. You may need to sign the clinic's own agreement. You pay the clinic directly for every order (medication, provider visits, shipping). You set your own prices. Licensed providers make every medical decision, and prescriptions are filled by licensed 503A or 503B pharmacies. The clinic may approve, pause or end your clinic access under its terms.
  4. Your responsibilities. You won't diagnose, prescribe, dose or give medical advice. You won't make medical or income claims in your marketing, or sell "research use only" products. You'll protect your clients' health information and keep your own business licenses current.
  5. No guarantees. We don't promise income, clients or any specific result. Examples and calculators are illustrations, not promises. Your results depend on your market and your effort.
  6. Education only. We're coaches. We're not your doctor, attorney, accountant, tax advisor or investment advisor. Wealth content is general education, not personalized investment or tax advice. Any insurance product is separate and never required.
  7. Our content. Programs, recordings and templates are licensed to you for your own business while your membership is active. Don't share, resell or copy them. Calls may be recorded and shared with members.
  8. Case studies. We may share your wins, results and feedback as case studies and examples in our coaching and marketing, including your name, business name, city and photo. We'll only share real results. To opt out at any time, email [email protected] and we'll stop using them in anything new.
  9. Independent business. You run your own business under your own name. You're not our franchisee, partner, agent or employee, or the clinic's.
  10. Ending the membership. We may end your membership for non-payment or a breach of these terms. We may also end it for any reason with 30 days' notice.
  11. Communications. We may contact you by email, phone and text about your membership. Reply STOP to opt out of texts.
  12. Hold harmless. You agree to defend, indemnify and hold harmless The Profit Lifestyle, its owners, coaches and staff from any claims, losses or costs (including attorney fees) arising from your business, your marketing, your clients, or your breach of these terms.
  13. Liability and law. Our total liability to you is limited to what you paid us in the last 3 months. Arizona law applies, and any dispute is handled in Maricopa County, Arizona.
